Children Ages 5-11 Vaccination Clinics 11/18/21
The Town of Hanover, in collaboration with Brewster Ambulance, will be hosting a Pfizer COVID-19 Vaccination Clinic for children ages 5 through 11.
- Dates: December 1, 2021 (first dose) and December 22, 2021 (second dose)
- Time: 4:00 pm to 7:00 pm
- Location: Council on Aging, 665 Center Street, Hanover, MA
Registration
A parent/guardian must attend with their child. To register for the clinic:
- Sign up for the Pfizer COVID-19 Vaccine Clinic for 5 - 11 Years Old, December 1, 2021
- Download, print two copies of each, and complete both the registration form and the consent form (2 forms, both sides)
- You will need to bring both forms to both clinics
Because the dose for 5 through 11-year-old children is smaller than for older children and adults, there will not be vaccines available for those over 12 years at this clinic. If you have any questions about the vaccine, please refer to the CDC website, or consult your child's healthcare provider.
If you have any difficulties with the registration process, you may attend the clinic without registering and a staff member will assist you.
