Medical Requirements for New Hanover Families
Welcome to the Hanover Public Schools and the Office of Family and Community Engagement
2024 Summer Enrichment Programs!
The Health Services department would like to share important information about health requirements for our programs. All students that participate in our programs must have the following documents on file with the Summer Health Services Office:
- Required for All Students: A physical exam report from physician
- Required for All Students: A current immunization record (typically included in the physical exam report) from physician
- Required for All Students: Confidential Student Health Information form
- If Applicable: Authorization for Medication Administration in School form Note: this form requires a physician signature
- If Applicable: Food Allergy & Anaphylaxis Emergency Care Plan form Note: this form requires a physician signature
Please return Health Services forms by mail or in person by June 30th. Copies via fax or email will not be accepted for confidentiality reasons.

Authorization for Medical Administration in School
It is preferable that your child be given any needed medication at home. However, if medication is required while at the program, please bring the medication in the original pharmacy container with the child’s name, the name and dose of the medication and the prescriber’s name as well as a completed Authorization for Medication Administration in School form with physician and parent/guardian signature. All medication must be delivered to the school nurse by a parent/guardian or their designee.
Food Allergy & Anaphylaxis Emergency Care Plan
If your child requires an EpiPen, please have your child’s health care provider complete the Food Allergy & Anaphylaxis Emergency Care Plan (or equivalent) and ensure both the parent/guardian and physician/prescriber sign the form. The Epipen should be delivered to the nurse by a parent/guardian or their designee. Requests for student self carry should be discussed with the nurse on the first day of the program.
Sunscreen & Insect Repellent
Sunscreen and insect repellent should be applied at home, if needed. If your child needs to re-apply sunscreen or insect repellent while at our programs, please send the product in the manufacturer’s original container labeled with the child’s name, and indicating whether the student will self apply or the student will self apply under the supervision of the school nurse and also must include instructions on when to apply.
