Center School Placement Grade 2 into 3 and 3 into 4

Attached you will find a link to the student information sheet for parents/caregivers to complete if they would like to share information about their child as we plan for placement for the 2020-2021 school year. Please include your child's current grade and current teacher on this form. Once you have completed the form, please click submit and it will be sent directly to the Center Office. Please return your response by Friday May 15th
