Hanover Middle School Grade 6 Camp Squanto Medications Information 3/22/23

Information for Grade 6 Parents/Guardians Regarding Medications at Camp Squanto

The school district requires the parent to provide a written physician’s order along with signed parent consent if your child takes any medication that will need to be given at camp. View and download the Authorization for Medication Administration in School form.

It is necessary that the parent or guardian hand deliver all medication to the school health office in the original prescription bottle but containing only the number of pills that will be taken during the time at camp. A Doctor’s prescription or order IS required.

Medications will be accepted June 1 - June 8, 2023.

If your child takes an over-the-counter medication or supplement on a daily basis, such as Sudafed, Claritin, Zyrtec, melatonin, etc., you are requested to provide only the number of pills needed during camp, and they too must be in the original container. No loose pills are accepted. As mentioned above, you will have to provide a physician’s order.

In the event that your provider has their own forms, you may attach them to this form and complete just the parent portion.

Medications that will not require additional forms are Tylenol, Ibuprofen, Diphenhydramine (Benadryl), Bacitracin, and Hydrocortisone. These medications are available in the health office at camp as needed.

Questions regarding any of the above or health concerns while your child is at camp, please call the Hanover Middle School Health Office at 781-871-1122, ext 1.

Sincerely,
