Hanover Middle School Washington D.C. Trip Medication Drop-off 5/23/23

The school district requires the parent to provide a written physician’s order along with signed parental consent if your child takes any medication that will need to be given at any time during the field trip. I have attached a link to the form required for your convenience.

It is necessary that the parent or guardian hand deliver all medication to the school health office in the original prescription bottle but containing only the number of pills that will be taken during the time at camp. A Doctor’s prescription or order IS required. 

If your child takes an over-the-counter medication or supplement on an as-needed or daily basis, such as Dramamine, Sudafed, Claritin, Zyrtec, melatonin, etc., again, you are requested to provide only the number of pills needed during the trip, and they too must be in the original container. No loose pills are accepted. As mentioned above, you will have to provide a physician’s order.

I will begin to accept medications between 7:15 am and 2:00 pm, starting Monday, May 22, 2023, and ending Wednesday, May 31, 2023. Dropping off medications as early as possible is greatly appreciated, as I would like the opportunity to organize them prior to our field trip. I will be available to accept medications at the information night on Wednesday, May 24, 2023.
