Preschool Registration

2024-2025 School Year

Our current preschool programs are four days a week (Monday, Tuesday, Thursday, and Friday) with both AM and PM sessions. Our integrated program educates students both with and without disabilities. Three and four-year-old children attend the morning class and four and five-year-old children attend the afternoon class.

As part of the registration process, you will be asked to complete and upload the following registration forms.

  • Cedar School Parent Information Form.
  • Medical History Form.
  • Developmental Questionnaire.
  • Primary Home Language Survey.
  • Registration Form.

Program Description

  • A tuition-based 4-day preschool program (Monday, Tuesday, Thursday, Friday) with morning and afternoon sessions.
  • Parents are required to provide transportation.
  • Peer models must be toilet trained before the start of school.

Medical Policy

The medical requirements for Preschool admission are identified as:

  1. A full physical exam, including a vision screening, by your family doctor/pediatrician within twelve (12) months before the start of school.
  2. All students must be completely immunized according to the Massachusetts School Immunization Law as follows:
  3. Massachusetts School Immunization Requirements.
  4. Blood Lead Level, including level amount and date.
  5. A physician's certification is the only acceptable evidence of immunization. Certification provided should contain the month, day, year, and type of immunization.

There are two exceptions to these laws: 

  • For medical reasons.
  • For religious reasons.

However, these will not be accepted as valid unless accompanied by:

  • Official Medical Statement with reasons for Medical exception with doctor's signature.
  • Parental/guardian statement that immunization conflicts with sincere religious beliefs.

Children will not be allowed to attend school without immunization documentation.

  • If your child has had any vision or hearing testing done either at his/her regular doctor or a specialist, please include those results with his/her physical forms.
  • If your child has Asthma or a Severe Life-Threatening Allergy requiring an Epi-pen; please see the School Nurse for additional forms.
  • If your child will require any medications to be given and/or kept at school, please see the School Nurse for additional forms.

Screening

Additional information will be provided at a later date and sent to the email address provided at registration.
